By signing above, each of the above-listed business and business owner (individually and collectively, "you") authorize Kapitol Bridge ("FAC") and each of its representatives, partners, successors, affiliates, contractors, designees, and assigns (referred to individually as a "recipient" and collectively as the "Recipients") to obtain consumer or personal, business and investigative reports and other information about you, including but not limited to credit card processor statements and bank statements, from one or more consumer reporting agencies, such as TransUnion, Experian and Equifax, and reports from other credit bureaus, banks, creditors and other third parties.
CONSENT TO TELEPHONE CALLS AND TEXTS: You expressly consent to receiving marketing and other calls and messages, to landline, wireless or similar devices, including auto-dialed and prerecorded message calls, and SMS messages (including text messages) from Recipients, at telephone numbers that you have provided and you waive any claim under the TCPA or related state equivalents. Message and data rates may apply. Your consent to receive marketing is not required for your application; if you do not consent do not provide phone number.
CONSENT TO ELECTRONIC DISCLOSURES: You expressly consent to transactions and disclosures with Recipients online and electronically. Disclosure will be provided to you either on the screen, on Recipients website or via electronic mail to the email address you provided.
WITHDRAWING CONSENT: You may withdraw consent for telephone calls or electronic disclosures by calling FAC's Customer Service (563)-856-1825 or emailing info@kapitolbridge.com
